PCI Vendor Request

Pacific Charter Institute (PCI) is currently accepting applications for vendors to participate as an active service provider for its students. The final day to apply to become an approved vendor for the 2016-2017 school year is Friday, March 31, 2017. All vendor applicants must be referred by a teacher, student, parent, or staff member of PCI and its entities of Heritage Peak Charter School, Rio Valley Charter School, Sutter Peak Charter Academy or Valley View Charter Prep. All PCI vendors are required to be either compliant with AB 1610/Education Code Section 45125.1 or fingerprinted by the Department of Justice (DOJ) to clear a background check using a PCI issued DOJ Request for Live Scan Service form. Being an Active Vendor with PCI will allow the vendor to bill PCI and its entities for the services provided to students by way of Approved Vouchers and Invoices. Services must take place between the first and last days of the school year.

Online Applications

New Vendor Applicants

If you are interested in becoming an Active Vendor with PCI, please complete the PCI New Vendor Application to be considered a PCI Vendor for the 2016-2017 school year. Please have the following documents readily available to upload as an attachment:
 
  • Completed W-9 Form (an electronic copy of the form can be found on the right under Documentation)
  • Insurance Policy Accord or Coverage Summary (if applicable)

Returning Vendors

All approved vendors from 2015-2016 must renew their vendor status each school year to remain on the Active Vendor List and acknowledge the updates to the protocols and requirements for the school year. Vendor renewals will be available on the 1st of July prior to each school year. If you are an approved vendor and wish to remain as an active vendor with Pacific Charter Institute, please complete the PCI Vendor Renewal Form. Please have a copy of any updated documentation readily available to upload as an attachment.

  5. EC Statement of Compliance (if you are a Custodian of Records with the DOJ or have an ORI number to fingerprint your employees)

    a written request for DOJ Request for Live Scan Service Form along with a list of employees
